With over 19 years of nursing experience, Aurea brings a wealth of knowledge and compassion as our Director of Case Management. She has extensive experience in Staff Development, Skilled Nursing Care, Hospice Care, Case Management, Urgent Care and Critical Care Services in the Intensive Care Unit (ICU). Her diverse expertise and dedication to patient-centered care ensure that every client receives personalized, high-quality support tailored to their unique needs.

Case Management Supervisor

Rhealyn Perez, RN BSN, is a highly skilled Nurse with a broad background in nursing and clinical leadership. Her expertise spans from Skilled Nursing Care, Hospital Acute Care, Residential Care Facility for the Elderly (RCFE), Congregate Living Health Facility (CLHF), Home Health, Hospice Clinical Management, and Concierge Nursing. With her commitment to delivering exceptional patient care, Rhealyn ensures that each client receives compassionate, individualized attention in every setting she serves.

They conduct an in-depth assessment of clients' health and care requirements, coordinating with healthcare providers, families, and caregivers to develop a customized care plan. Our Case Managers also oversee the management of caregivers, ensuring they provide compassionate and professional support while adhering to the care plan.
